OGE Forms 450, 278 and 278-T are
provided below in various versions that are suited to individuals with different
reporting requirements. Please read all instructions fully before choosing
the form appropriate to your requirements. You
must download these forms to your PC prior to using. Changes made directly
to the on-screen version cannot be saved and will not print correctly.
OGE FORM 278 (PUBLIC FINANCIAL DISCLOSURE REPORT) (December 2011 Edition: Previous editions are no longer authorized.)
The OGE Form 278 is provided below in two versions, suited to individuals with different
reporting requirements. Please read the OGE Form 278 instructions fully before completing the form appropriate for your employment requirement.

- Annual
Post-Employment Certification: Subsection 8-400 of DoD 5500.07-R,
Joint Ethics Regulation, requires members of
the Department of Defense
who file a public financial disclosure report (OGE 278) to certify each year
that they are aware of the restrictions that three statutes place on them
during Federal service when negotiating employment, and after departing Federal
service. They must also certify that they have not knowingly violated these
statutes. This Annual Post-Employment Certification accomplishes that task.
It also meets the required notification to senior officials of post-Government
employment restrictions under 18 U.S.C. Section 207.
OGE FORM 450 (CONFIDENTIAL FINANCIAL DISCLOSURE REPORT)

Pages). Supervisors and reviewers hold a legal obligation to fully review the financial disclosure reports
of their employees. It is critical that you read through the instructions for each part of the report,
gather your financial records, and then fill out the form. You are responsible for ensuring that the information you submit is complete and correct before submitting it to your supervisor for review.
IMPORTANT: Do not send your completed OGE
Form 450 directly to the SOCO Office. Your agency is responsible for reviewing each report
for administrative sufficiency (proper position titles, phone numbers, completed
entries and proper signatures) for each of their employees. Contact your agency
administrative office to obtain the correct address for distribution or mailing of
your form.
